How do I know if The Lazar Center is right for my child?

Effective therapy is often reliant on a strong positive relationship between the therapist and the patient. Dr. Lazar offers a 10-minute complimentary informational phone consultation for introductory purposes. Please feel free to reach out to see if the Lazar Center’s services are the right fit for your child.  

Does The Lazar Center take insurance?

The Lazar Center is an out-of-network provider and will provide you with a superbill receipt that can be submitted to your insurance provider for reimbursement.  Many clients find that services are covered through their out-of-network benefits. 

CPT codes to inquire about are: 90791 (intake), 90834 (individual), 90846 (without patient), 90847 (patient + family), and 90853 (group).

When is payment due?

Payment is due at the time of each visit.  The Lazar Center accepts checks, cash, Zelle®, or major credit cards. The Lazar Center can provide you with a receipt to submit for reimbursement from your insurance provider.

What happens if I need to cancel an appointment?

Except for emergencies, The Lazar Center requests 48 hours advanced notice to cancel or reschedule sessions, or the client will be responsible for the full session fee.
